So what's the line for humanizing characters?

Aphra here is a young-ish (Lovecraftian) Deep One (though in her particular take on Lovecraft, they are a human subspecies). Right now, she notes in her canon that to people who don't know her background, she looks like an ugly woman (her words; it bothers her that nearly everyone she lives with has a different standard of beauty that excludes her). She only gets obvious gills and scales and such a few decades down the line.

Does she still look like herself since she passes for a normal human, or does she get the dysphoria of looking like a 'woman of the air' (read: standard human). Or is it player choice?
